Investigation Summary
On February 25, 2008, Employee #1, a sanitation worker, was standing on an 8 ft ladder cleaning a high speed roll-up door. A coworker tripped the electronic photo eye causing the door to become energized. The door pulled Employee #1's right arm into the roller cylinder. Employee #1 was hospitalized with a fractured arm.
