Investigation Summary
On July 3, 2007, Employee #1, the plant operations manager, was cleaning the discharge chute of a Manzini Finisher. The finisher was in operation when Employee #1 reached into the chute, approximately eight inches, to ensure that the chute was clean. His left index finger came in contact with the rear rotating impeller, amputating it. The machine is plumbed with a water injection system for cleaning and there are established practices for use of a large diameter hose to flush the chute. Employee #1 did not follow the lockout/tagout procedures. He was not hospitalized for his injury.
