Accident Investigation Summary
Summary Nr: 201089703Event: 05/31/2007Employee Is Injured In Fall Through Roof
On May 31, 2007, an employee was crossing the field while carrying a sheet of plywood to reinforce the east perimeter edge. The sheeting failed causing the employee to fall twenty-two feet on the concrete floor below, resulting in fractures of transverse process of L1, L2, L3 on the left side, transverse fracture of left humerus, fracture of left ankle. The employee was hospitalized for 10 days and underwent surgical repairs. Initial survey of roof identified weak and deteriorated sheeting on the east side perimeter of the roof, although defects were not identified in the field. Existing roof covering was reported to have been removed from the area of failure prior to employee's fall.
