Investigation Summary
At approximately 4:30 p.m. on June 20, 2008, an employee, a teacher, was using a three feet ladder, step stool, to hang informational material on a bulletin board in the classroom. The employee could not reach an end from the ladder. Hence, she placed her foot on a wooden shelf nearby to stretch and reach to complete the task. While doing so, she lost the footing on the shelf and fell to the class room floor. The employee suffered multiple fractures of the right leg. The employee was taken to the Tri-City Hospital for treatment. The employee was hospitalized and treated for fractured leg. The ladder involved was a metal step stool type three feet ladder.
