Investigation Summary
At approximately 7:00 p.m. on May 8, 2008, Employee #1 was a miter saw operator and a temporary employee. He was struggling to adjust a 3 to 4-in. piece of raw material in the feeding side of a double bladed miter saw, Model D45 AX, Serial Number 1490. The piece kept blowing out of place, and he tried to use tweezers. He used his right fingers to place the raw material into the point of operation of the saw blades when he tapped the foot pedal. This caused the dual saw blades to activated. His right index finger was caught by the pneumatic holder. The pneumatic holder held his right index finger up against the rail of the front fence as the two saw blades completed their full cycle. The saw blades amputated his right index finger, and he was hospitalized.
