Investigation Summary
At approximately 8:30 am, on January 25, 2008, Employee #1 , a foreman of E.C. Labor Inc., sustained an extruded right side lumbar disc as a result of her stooping over while cutting lettuce. Employee #1 was stooping over cutting lettuce when she felt a severe pain in her right leg that caused her to stand up and blackout. The next thing she remembers is waking up in the ambulance. Employee #1 was transported to Pioneer Medical Hospital in Brawley California. Employee #1 had surgery on January 27, 2008 to remove extensive fibrotic disc material of the entire right one half of the L4-5 space.
