Investigation Summary
At approximately 6:00 p.m. on December 30, 2007, Employee #1, a dishwasher, grabbed a stack of plates for delivery. Employee #1 slipped on something on the floor and fell forward, striking his chin on the kitchen floor and breaking a number of the plates. One of the sharp edges amputated the tip of the little finger on his left hand. There are floor mats around the cook line and the dishwashing line, but not in the walkways in between. He had both large dinner plates and smaller side plates. Employee #1 was transported to a hospital, where he received skin grafts during a 3-day hospitalization. The employer reported the accident on January 4, 2008, at 11:20 a.m.
