Employee's Finger Is Amputated in Machine Drive Mechanism

On October 1, 2007, Employee #1, a sanitation operator at a corn and flour tortilla and chip manufacturing facility, was asked to clean an uncooked tortilla cooling conveyor machine drive mechanism. The machine was running, and the door/cover for the mechanism was open while Employee #1 was attempting to clean that specific area. His left finger was caught in between the chain and sprocket drive system and amputated to about "2 1/2 digits." Employee #1 was performing the assigned task alone during the accident. The injury occurred at approximately 5:50 a.m., and the employer reported the injury to the Division at about 10:00 a.m. on October 1, 2007. The employer did not have a manual for the machine or an effective lockout/tagout program. The machine was not locked out for cleaning.
