Employee Is Struck by Forklift and Fractures Ankle

At approximately 8:00 a.m. on July 30, 2007, an employee was unloading a trailer and was waiting at the middle of the warehouse to find and place the merchandise. As the forklift Toyota, (Model Number 4607, Red color, propane operated) was backing on the dock at a warehouse; Employee #1 was walking by and was run over. The rear of the industrial truck made contact with Employee #1, and he fell to the ground. The other employee did not see Employee #1 and was still proceeding in reverse, during this time the rear wheel seriously injured Employee #1's left foot (ankle). Employee #1 sustained a fractured ankle and was hospitalized.
