Investigation Summary
At approximately 9:30 a.m. on August 9, 2007, Employee #1 was cleaning the top of a cabinet in a classroom. She was standing on a 36-in. high counter and was using a plastic classroom chair as a stepstool to get down. The plastic chair broke when the employee stepped on it, causing her to lose her balance. The employee fell and struck her head and body on the counter as she fell to the floor. A coworker heard the accident and called for emergency medical assistance. The employee sustained a punctured lung and two broke ribs and was transported to Palomar Hospital in Escondido, California. Employee #1 was hospitalized.
