Investigation Summary
At approximately 3:05 p.m. on June 27, 2007, Employee #1, a floor installer's helper was using a 10 in. Ryobi table saw, BTS 10, serial #063833456, to cut wood for a floor at a single-family residence. He was cutting a board when the saw struck his left hand. Employee #1 sustained amputation of his index finger and half of his middle finger, and a deep laceration to his thumb. He was taken to the Palomar Medical Center, where he was treated and released the next day. The employer provided workers with basic PPEs such as masks, gloves, ear plugs, safety eye wear, and hard hats, but employees are expected to purchase their own tools. At the time of the accident, the saw was operating at 4,800 rpm.
