Employee Fractures Arm When Caught in Truck Bed Gate

At approximately 11:55 a.m. on March 23, 2007, Employee #1 was operating a forklift (Komatsu forklift Model Number FG25TLP-12, Type LP, Serial Number 510453A) and was unloading a mud tub from a truck bed. Employee #1 was looking forward instead of backwards and got his left arm caught on the wooden gate of the truck bed. Employee #1 sustained a broken left arm with two broken bones. Employee #1 was hospitalized for16 days.
