Investigation Summary
At approximately 1:30 p.m. on Monday, February 26, 2007, Employee #1 was working as a warehouseman at a distribution warehouse for greenhouse components in Oceanside, CA. The business includes a small shop area where some minor manufacturing work is done as well. Employee #1's job that day included stacking metal poles onto a rack to be banded. To have the rack better fit the poles, Employee #1 decided to cut a 16-inch-long piece of 2 by 4 pine stud on a Ryobi table saw that was located in the shop. Although he had experience in high school and felt that he knew how to operate the saw, he had not used the table saw before and had not received training in how to operate a table saw. He made two 60 degree crosscuts at either end of the stud and prepared to rip the wood piece in half. Using neither a push stick nor the fence on the saw, he held onto the wood and fed it into the saw. There was no blade guard on the saw. Near the end of the cut, the wood bound up in the blade and kicked out, dragging his right hand over the table saw blade. Two fingers of his right hand were cut by the blade. Employee #1 was transported to Scripps La Jolla Hospital, where he received treatment for lacerations and underwent surgery to reattach his right hand ring finger, which was nearly amputated. He was not hospitalized.
