Investigation Summary
On April 3, 2007, an employee was removing the chains and straps holding 2 pipe racks and 4 tiers of pipe onto the pipe truck. The pipes were within the pipe bunks but the racks were sitting on top of the pipes. One rack was sitting upright and the other was inverted and then chained to the other rack. The employee was the driver of the truck and the passenger was his mentor. Each got out on either side of the truck and as they began to loosen the chains and straps that secured the load, a coworker climbed on top of the load and removed the chain that held the two racks together. When he had taken the chain off, the inverted rack rotated to the driver's side and struck the employee on the head and killed him.
