Investigation Summary
Employee #1 was adjusting pieces of dunnage on a forklift. The load was in the air, the parking brake was set, and the forklift was running. The forklift moved forward and pinned him against an adjacent stack of lumber. He was crushed and killed. Further investigation revealed that the employee either was not aware or forgot that the parking brake was not working.
