Event: 12/16/2008
Employee Is Killed When Loader Is Driven Into Pond

Employee #1 and two coworkers were removing snow at a chemical plant. Employee #1 was operating an ASV track-type skidsteer loader, Model Number RC60, Serial Number RSE01776, and was clearing snow around tanks and other areas of the plant. The employees agreed that when they were finished, they would meet in the contractor parking lot of the plant. When Employee #1 did not arrive, the coworkers went looking for him. They discovered tracks and saw a large dark hole in the mud and ice approximately 40 yards out into a sludge pond. Emergency services were contacted and Employee #1's loader was recovered from the sludge pond at approximately 2:00 a.m. on December 17, 2008. He had drowned.
