Investigation Summary

Investigation Nr: 200271849
Event: 03/20/2008
Employee Is Killed When Crane Overturns

At approximately 7:40 a.m. on March 20, 2008, Employee #1 was operating a Broderson Industrial Crane, Model Number IC-200-3F, to lift two steel plates with a combined weight of approximately 14,050 pounds from a flatbed trailer. The crane was aligned parallel to the flatbed. The length of the boom was 32 ft and the angle 55 degrees, which resulted in a load radius of 17 ft. Using this configuration and accounting for the stowed jib, which weighed 400 pounds, the capacity of the crane was 9,160 pounds (18 ft radius allowed for a pick of 9,560 pounds less 400 for the jib). With this crane load capacity, the crane was overloaded by 4,890 pounds. As the load was lifted, the crane overturned, crushing the crane cab into the side of the trailer and killing Employee #1.
