Investigation Nr: 201344645
Event: 11/07/2007
Employee Is Killed When Crushed by I-Beam

At approximately 1:30 p.m. on November 07, 2007, an employee was operating a 40 ton overhead crane. The crane was operated by pendant control in Load out Bay B of the plant. The employee was removing three steel 66 foot long I-beams from a load out trailer to relocate the beams to the painting area to be painted and shipped to a customer. A coworker was walking near the accident site and noticed that two of the steel beams were tipped over into each other and did not see the employee until he got right over the tipped I-beams. The employee was found pinned between Beam #3 and Beam #2. The coworker immediately called for assistance, and the employee's Supervisor called emergency medical services. There were no witnesses to the accident; employee was working alone. Employee was transported to Ransom Memorial Hospital, Ottawa, Kansas, where he was pronounced dead.
