Investigation Summary
On May 31, 2007, an employee was working in the head house, setting the travelling tripper "shuttle" and performing general house keeping activities. The employee's right hand was caught between the belt and pulley of the tripper. The employee was able to free the right hand by pulling it out with the left hand. The left hand was then caught between the belt and pulley, pulled around the unit and caught between the side of the pulley and the tripper frame. When the left hand was caught, the employee's face struck the pulley, resulting in broken jaw and lacerations to the face. The employee was hospitalized and treated for amputation of both arms and face injuries.
