Investigation Summary

Investigation Nr: 201344389
Event: 03/22/2007
Employee's Arm Is Amputated When Caught in Machine

At approximately 7:45 a.m. on March 22, 2007, Employee #1 was working in a maintenance machine repair shop, building Number 46, of an aircraft company. He was cleaning the shaft of the Tumbla-Burrer machine using the Summit-H Lathe; machine Number 1116, the sanding paper roll style Emory cloth. A coworker, approximately 10 ft away, only saw Employee #1's back and did not know what he was doing. Employee #1 was cleaning something off of the lathe with a rag. The rag caught in the lathe and pulled his arm into machine before he could hit the emergency stop. Employee #1 lost part of his left arm and was hospitalized.
