Investigation Summary
At approximately 3:40 p.m. on February 26, 2007, Employee #1 and a coworker were working in the building doing demolition and removing debris from the premises. The employees decided to move a cart that had a section of tower nestled on it. The cart was located near an overhead door. The tower section alone weighed approximately 2,800 pounds. As the employees began to push the cart they noticed both tires on the cart were flat. They used a Hyster H80XL, Model Number 3107 forklift to lift the cart so they could service the tires. The cart was lifted approximately 2 in. to relieve the tire pressure. The coworker was called away after airing up the south tire; he told Employee #1 to finish up airing the other tire. Employee #1 decided to lift the cart more. The cart was raised approximately 2 ft off the ground and also had the forks tilted back. As Employee #1 reached over the forks to air up the tire, the top-heavy tower rolled over onto him. Employee #1 was standing between the load and the back of the forks when the accident occurred. Employee #1 was taken to the hospital where he died from asphyxia as result of crushing injuries to the chest and abdomen.
