Employee Falls Striking Head, Later Dies

At approximately 7:00 a.m. on December 12, 2008, Employee #1 was attempting to tie his shoe with one leg up on a table. He fell backwards into a refrigerator striking the back of his head. He worked through his shift and clocked out at approximately 2:00 p.m. that day for regularly scheduled leave. Later that evening, Employee #1 reported a headache to his wife. He was taken to a hospital and died on December 13, 2008. %09
