Investigation Summary
At approximately 12:30 p.m. on February, 22, 2007, Employee #1 was lowering a cutting torch on the west side of the bridge. The cutting torch was being lowered over the side of the bridge so that an employee below could cut off bolts that had been used to hold up pier scaffold sections. Employee #1 was not wearing fall protection at the time. Employee #1 lost his balance and fell 19 ft to the river embankment and suffered a concussion. Employee #1 was killed.
