Investigation Summary
At approximately 3:00 p.m. on May 30, 2007, an employee was operating Gradall forklift to transport an air compressor from the south to the north side of the project. He installed a nylon sling on an air compressor lifting eye and a polypropylene rope connected to the tongue of air compressor and used it as a tag line during one-person operation. The tag line was used to keep the air compressor from spinning around while hoisted off the ground. He was holding the tag line and some portion became wrapped around his left ankle. Upon reaching the north side of the project, the employee lowered the air compressor to the ground. The employee released the tag line and backed up the Gradall to slip the nylon sling off the fork. While backing up, the Gradall forklift ran over the rope tag line and stretching it forward, making the employee's left ankle became twisted, which resulted into a severe ankle and leg injury. The employee was transported to the El Camino Hospital, which was 300 meters way from job site by a coworker. The employee underwent surgery and stayed in the hospital for five days.
