Investigation Summary

Investigation Nr: 202495305
Event: 12/19/2006
Employee Crushed Between Trailer Load and Cab and Killed

At approximately 11: 26 a.m. on December 19, 2006, Employee #1, an independent truck driver, was preparing to remove a conveyor assembly from a construction site at the San Jose Mineta Airport. The unit had already been set on the bed of a flatbed truck, but had not yet been secured. Parked to the right of Employee #1's vehicle was a second rig carrying a conveyor unit that was attached and ready to go. Employee #1 was standing on his trailer between the conveyor assembly and the back of the tractor cab when the other truck began to move. As the other truck pulled forward, the rear of its conveyor assembly load caught the rear of the assembly on Employee #1's truck. The unsecured unit slid forward on the trailer and crushed Employee #1's head against the back of the tractor cab. Employee #1 was pronounced dead at 11:34 a.m. by the San Jose Fire Paramedics.
