Investigation Summary
On October 3, 2006, Employee #1 was assisting with repairing the rotating portion of a conveyor belt system. He was tasked with turning the "pins" that held together the leather sections of the conveyor belt. Another employee was at the controls of the conveyor and could not see Employee #1. Employee #1 was fixing one of the pins on the stopped belt and was injured when the rotating belt system started moving unexpectedly. He was hospitalized for a broken right arm.
