Investigation Summary
On December 31, 2007, Employee #1 removed a large split rim tire from a large reach truck used for unloading rail cars. The rim tire failed; the rim and tire exploded off the vehicle and struck the employee, who stood in front of the tire. Employee #1 was killed.
