Employee Is Burned, Later Dies after Release of Lpg Fuel

At approximately 12:45 p.m. on October 26, 2006, an employee was filling a 10 pound LPG cylinder for a customer. According to the customer, the employee placed the cylinder on the scale, activated the LPG tank's flow motor pump to begin the filling process, but failed to hook up the filler hose valve into the cylinder.
