Investigation Summary
At approximately 9:25 a.m. on February 27, 2009, Employee #1, a production manager, was working at a facility that engages in the receiving, storage, shipping, and recycling of new and used whole sale/ retail outlet books or magazines. Employee #1 was adjusting the tracking of a shifting 2 foot wide by 75-feet long Roach long Eastern conveyor belt Model Number BM232-3. After adjusting the conveyor belt, he began testing the conveyor by throwing several books and magazines on the belt. While testing the conveyor, a book measuring 1 inch by 4 inches by 6.5 inches fell through an opening that measured 14 inches long by 4 inches wide. This caused the machine to become jammed. Employee #1 attempted to clear the jam when his left hand was pulled into the machine. His left hand became lodged between the bottom moving belt and the tail pulley of the conveyor. Emergency services were contacted, and Employee #1 was taken to Arrowhead Regional Medical Center, California, where he was hospitalized and underwent surgery for the treatment of multiple fractures to his left forearm. The incident investigation revealed that the machine was not deenergized before Employee #1 attempted to free the jammed machine.
