Employee Crushes and Lacerates Finger in Printing Press

At approximately 6:40 a.m. on February 2, 2009, Employee #1 was operating a web printing press (SANDEN VARICOM, Model Number 1200) in slow mode (300 ft/min). Employee #1 was tightening a bolt with a 0.4375-in. box wrench, when the wrench slipped out of his hand and fell. Employee #1 tried to catch the falling wrench with his left hand, but his hand got caught between the moving ink and oscillating rollers, resulting in the crushing and degloving of his left index finger. He was driven to a clinic by Coworker #1. He was later taken to a hospital by paramedics and was hospitalized for more than 24 hours.
