Three Employees Are Injured in Motor Vehicle Accident

At approximately 10:10 p.m. on February 22, 2009, Employees #1, #2 and #3 were riding the gasoline-powered personnel carrier. E-Z-GO, Shuttle 6, Serial Number 925008, MFG code 80896, 15.2 hp, 13 mph maximum ground speed, identified as Number 459. The personnel carrier was riding was rear ended by a pickup truck, White 2003 GMC Sierra, CA License plate Number 7D79125. The personnel carrier was not equipped with taillights, brake lights, or turn signals. No emergency lights such as a rotating beacon or a flashing light were used. It was equipped with a single headlight. Natural illumination was effectively non existent since the accident which occurred at or around 10:10 p.m., occurred on a waning crescent moon phase with a 5 illumination on a partly cloudy night. On the other hand, some artificial illumination was provided by overhead road light fixtures. None of the employees were outfitted with reflectorized clothing or reflectorized material over clothing.. The pick up truck was traveling at an unknown rate of speed. All three employees were ejected from the carrier. The driver of the pick up truck was arrested by the CHP on the scene for suspicion of driving under the influence of alcohol. Employee #1 was transported to Arrowhead Regional Medical Center where she was hospitalized for leg amputation. Employees #2 and #3 were transported to Loma Linda University Medical Center, where they were evaluated and released.
