Employee Falls from Roof, Sustains Fracture to Wrist

On September 24, 2008, Employee #1, a roofing installer, was installing a metal roof on a pool house. He was leaving the pool house roof using an unsecured 12-ft aluminum extension ladder. The ladder feet (base) had been placed on disturbed soil, and as the employee started down the ladder, the base slipped down and out. The ladder tipped to the right, and the employee fell 5 ft. Employee #1's right wrist was broken, and he was hospitalized.
