Employee's Foot Is Crushed When Lost Control of Forklift

At approximately 1:00 p.m. on June 20, 2008, an employee was assigned to work as a forklift operator at the secondary employer's facility operating a reach truck forklift, Toyota Model Number 7BRU18, Serial Number 34427. While heading west at row J, the employee lost control of the lift truck as he approached the corner concrete wall and his left foot was smashed in between the back side of the lift and the wall. The employee was initially taken to the industrial clinic and was transported to the St. Bernardine Medical Center where he underwent surgery crushed left foot and fifth toe amputation and in-patient hospitalization for a period of three days.
