Investigation Summary
At approximately 2:00 p.m. on July 14, 2008, Employee #1 was operating an ACME, designated Number PW4, electric welding press machine, located in the Wire Department of a shelf manufacturing facility. He placed wires in the set-up mold die and was in the process of moving it with his right hand into the press welding zone when he accidently activated the foot pedal, pinching his middle finger. He was hospitalized with an amputated finger.
