Worker fractures thumb stacking machine

On November 09, 2007 at about 8:30 pm, a serious injury occurred at a place of employment. Employee #1 was operating a Bundle Splitter (Design & Integration Inc. Model B Split, S.N. 1518). Employee # 1 had been operating the machine for 4 hours prior to the accident and was using a remote control to operate the machine. Just before the accident, Employee #1 had a stack of about 30 sheets (1/8 inch thick) on a skid. The sheets were uneven. Employee # 1 was making the sheets even with his left thumb and same time another stack of (20-25 sheets) was coming towards him on the conveyor. The roll of the conveyor pushed the sheets on the stack on the skid and the Employee #1's left thumb got crushed between the back board and the top sheet of the stack. The accident caused multiple fractures on his thumb which required hospitalization for 4 days.
