Investigation Summary
At about 3:20 p.m.on August 08, a serious injury (amputation of finger) occurred at a place of employment.The employer is engaged in the manufacturing of concrete furnishing. At the time of accident Employee #1 was operating a miter saw(Delta Shopmaster, Model MS250, S.N. 357104Q, 10 in. Compound Power Miter Saw) to cut a piece of wood(2 in. by 6 in. by 10 ft). This was the first cut he made before the accident. After making the cut, he tried to remove the cut piece of wood and struck the rotating blade with his left hand and caused deep laceration on his left thumb, index finger and middle finger. Later his index finger had to be amputated.
