Investigation Summary
At approximately 2:30 p.m. on August 10, 2007, Employee #1, who worked for an underground cable construction contractor, was at a construction site when he noticed that a service truck towing a T-7 Ditch Witch trailer was starting to leave the work area. He saw that the metal holder of the e-vac end attachment on the trailer was about to run into a cluster of eight to nine 6 ft long by 1.63 in. in diameterdrill rods that were protruding from the back of a parked service truck. To prevent the collision, Employee #1 ran between the trucks to move the rods out of the way, but no coworkers knew he was there. The first service truck continued to move forward and struck the drill rods in the second truck, crushing Employee #1's left hand and amputating his fourth finger at the first knuckle. Paramedics provided emergency aid and ordered he be flown to Arrowhead Regional Medical Center. Employee #1 underwent surgery had surgery and was released that same evening.
