Investigation Summary
Employee #1, a field technician working for Time Warner Cable at 25994 Scenic Drive, Idyllwild, CA 92549, positioned a portable 28-foot fiberglass extension ladder (Werner, model number D7128) on a wooden pole in order to reach a cable's tab. The left safety shoe rested flat on the slopped asphalt ground; however, the right shoe was not held flat but in spur position in an attempt to achieve level positioning of the ladder. The ladder was not equipped with levelers. The ladder was equipped with a V-rung and two cable hooks (92/A92). The V- rung is the highest rung on the ladder and is designed as a single support attachment when the top support is a pole. The cable hooks are attached to the top of the ladder railings and are used to hook over cable strands. Employee #1 did not hook up to the cable strand; however, the ladder rested on the pole via the V-rung. Employee #1 proceeded to climb the ladder and when he reached approximately 12 feet above the ground, the ladder shifted to the right. Fearing that the ladder was about to fall, Employee #1 leapt off it and landed on the dirt ground. The ladder did not fall. Employee #1 was transported to Hemet Valley Medical Center where he was hospitalized for 4 days.
