Employee Fractures Pelvis When Struck by Falling Platform

At approximately 11:00 a.m. on July 30, 2007, Employee #1, a warehouseman, was working from a basket platform at an elevation of approximately 10 ft that was attached by chains to the forks of a "RAYMOND, Model Number DR30TT-EASi" High reach lift truck marked as Number 117. Employee #1 was straightening cardboard boxes on a pallet lying on the third-level rack shelf. As the lift operator was lowering the basket platform, one side got stuck on the rack edge causing it to slip and fall off the forks. The basket fell along with Employee #1 to the concrete floor. The basket hit Employee #1 on his left leg and pelvis causing multiple fracture injuries of his pelvis and left upper leg. Employee #1 was hospitalized for six days.
