Employee Leg Crushed When Pinned Between Pallet and Wall

At 6:00 p.m. on July 18, 2007, Employee #1 was working at the employer's distribution center. Employee #1 was operating pallet jack Number 414, Serial Number 6A153719, at the time of injury. Subsequent to the accident, the employer engaged the services of Crown Lift Truck for the purpose of conducting a mechanical inspection of the pallet rider Employee #1 was operating at the time of the injury. The mechanical inspection revealed that the brake hinge roller pin, a three part assembly, was broken. Also noted was that one of the three control handle springs was broken. The other two springs were worn. The Crown Lift Truck technician explained that this would have prevented the truck from stopping when the handle was raised. Employee #1 was a certified industrial truck operator, and he had completed the "Power Equipment Checklist." prior to using the electric pallet rider Number 414 on the day of the injury. Employee #1 did not note any deficiencies. Employee #1 sustained serious injuries to his lower left leg when it was crushed between the frame of the pallet rider he was operating and a wall at the loading dock. Employee #1 was transported to Arrowhead Regional Medical Center where he was admitted and hospitalized for seven days.
