Investigation Summary
At approximately 6:30 a.m. on May 18, 2007, Employee #1, a temporary employee, was working on a Whirlwind jump saw; Model 212, cutting aluminum strips 36-inch by 5-inch by 0.06-inch. As he picked up the cut parts left from cutting the strips, he reached under the guard, accidentally activated the saw by depressing the foot pedal. The running blade struck and amputated his right thumb, index and middle finger. He was hospitalized for five days.
