Investigation Summary

Investigation Nr: 202445391
Event: 05/02/2007
Employee Fractures Forearm When Caught Between Vehicles

At approximately 8:00 a.m. on May 2, 2007, Employee #1, a weatherization crew technician, was assigned as a spotter for a truck driver at the parking lot of the facility. When the truck driver saw a Suburban (another vehicle) coming down the driveway, the truck driver stopped and allowed the other vehicle to have the right of way before he make his turn. It appeared that when the truck driver stopped, Employee #1 walked around the truck to the right side towards the truck driver and lost eye contact with him. When the Suburban left the area, the truck driver continued to back up his truck and unknowingly pinned Employee #1 against the side of the parked vehicle at the north end of the loading and unloading dock. Employee #1 yelled at the truck driver to stop, but it seemed that the truck driver did not hear him and he continued to backup. Employee #1 pounded the side of the truck and that was when the truck driver stopped the truck. Employee #1 sustained a fracture of the left forearm. Employee #1 underwent surgery and was hospitalized for three days.
