Investigation Summary
At approximately 11:15 a.m. on May 11, 2007, an employee was walking to the restroom. At the same time, an operator was operating a RAYMOND DOCK STOCKER, Model Number R30-C30TT stand-in type fork lift, marked as Number 167 in the shipping dock area. The operator was moving the fork lift in a backward direction, when it ran over employee's right foot, causing serious foot crushing injuries. The employee was taken to Arrowhead Regional Medical Center for treatment and surgery. The employee was hospitalized for three days.
