Investigation Summary

Investigation Nr: 202446092
Event: 04/04/2007
Employee Is Injured When Struck by Falling Lumber

At approximately 12:15 p.m. on April 4, 2007, an employee, a laborer, was settling 2 by 4 stringers on the ground while a JCB LOADALL Model Number 520 Lift Truck operator, with a load of 48 4-ft by 8.375-ft OSB sheets on raised forks moved the lift truck to set the load on the stringers. When the lift operator stopped the machine, the OSB sheets slid off the forks and landed on top of the employee. Coworkers lifted the OSB sheets off the employee. Paramedics took the employee to the Inland Valley Regional Medical Center, Wildomar, California for emergency medical care. Medical reports indicated that he suffered a right ankle fracture, a broken left leg along with injuries to his right hand and lower back. He was admitted for surgery and hospitalized for five days.
