Investigation Summary
On October 11, 2006, Employee #1 and a coworker were at a coal-fired, electrical power generation plant, planning the renovation of a large inlet condensor water box. The plan was to remove the unit from the plant during a scheduled multi-week maintenance shutdown, and bring it to a sandblasting area where they would pressure-wash, sandblast, and paint the box. Before beginning the job, Employee #1 thought the tall, 6,900 lb water box looked unstable. It was sitting on 12 in. by 12 in. wooden cribbing blocks situated on two large I-beams that were laying on the ground. He was climbing onto the water box to check its stability when it fell over on top of him. Employee #1 was crushed and killed.
