Employee Dies When Crushed by Truck Lift Gate

At approximately 3:45 a.m. on September 9, 2006, Employee #1 and a coworker, both truck drivers, were dispatched to make deliveries to a vehicle dealership. After loading the truck, Employee #1 was standing on the ground at the rear of the truck in front of its Waltco lift gate, Model Number E-25RM/E-30RM, in order to either set the wheel brake or adjust the wheels. As he was making the adjustments, the parts cage rolled off of the platform and struck Employee #1. Employee #1 was pinned to the ground, resulting in a severe head injury. Emergency services were contacted and Employee #1 was taken to Presbyterian Hospital in Pittsburgh, PA, for treatment. Employee #1 died 21 days later. The accident investigation revealed that the wheel brakes were inoperable.
