Employee Amputates Finger Tip While Securing Material

On June 6, 2008, Employee #1 saw that the material a forklift was moving was going to become wedged. Employee #1 reached in with his hand to prevent material from becoming wedged. Employee #1 sustained a left index finger tip amputation. Employee #1 was not hospitalized.
