On July 3, 2008, Employee #1, a roofer, climbed an extension ladder to gain access to a steel canopy approximately 13-feet high. The extension ladder displaced and Employee #1 fell approximately 13-feet to the cement floor, resulting in serious injuries and hospitalization.
