Investigation Summary
On February 27, 2008, Employee #1 was operating a forklift on a portable loading ramp. As he started-up the ramp, the ramp slipped away from the truck, and the forklift fell off the top of the ramp. As the forklift fell-off the ramp, the employee unbuckled his seat belt and tried to jump off the forklift. While the employee was jumping out of the forklift, the forklift struck his lower left leg, fracturing it. Employee #1 was hospitalized. The employee was found clear of the forklift. He did not position the ramp properly and had not attached both chains to the truck.
