Investigation Summary
While driving a fork lift along a dirt road in a watermelon field, Employee #1 hit a "pot hole", causing the forklift to begin bouncing and become airborne. Employee #1 was not wearing his seat belt and was ejected from the forklift. The forklift ran over his head resulting in his death.
